Governance, Defensibility, and AI in Fitness for Work Screening

As AI-supported screening systems become more common in workforce health, medical leaders are asking the right question: 

Does this strengthen or weaken clinical governance? 

The discussion should not be about automation. It should be about defensibility. 

For occupational physicians and Chief Medical Officers, the objective is clear: consistent, explainable, and accountable fitness for work determinations. 

Any decision system should be evaluated against that standard. 

 

The underlying issue: unwarranted variation 

One of the most persistent challenges in fitness for work screening is variability. 

Two clinicians reviewing the same disclosures may reach different conclusions. Differences in risk tolerance, workload pressure, interpretation of role demands, and local context can all influence outcomes. 

At small scale, this may go unnoticed. 

At enterprise scale, variation becomes difficult to defend. 

Reducing unwarranted variation while preserving escalation pathways is a legitimate governance objective. 

Decision systems should be assessed on whether they improve consistency without removing clinical authority. 

 

Digitisation is not the same as decision quality 

Moving from paper to digital forms improves documentation and accessibility. It does not automatically improve decision quality. 

Defensibility depends on structured decision logic, clear role context, and defined escalation pathways. 

If those elements are inconsistent, the medium does not matter. 

Clinical governance requires more than data capture. It requires consistent application of risk thresholds. 

 

Static rules versus adaptive learning 

Rules-based screening systems apply predefined thresholds consistently. This reduces reviewer drift but relies on assumptions embedded at the time the rules were written. 

Adaptive systems introduce a different mechanism. By learning from large volumes of clinician-reviewed determinations, they refine how risk factors are weighted over time.

A defensible model includes: 

  • documented escalation pathways 
  • clinician review for complex cases 
  • transparency in how recommendations are generated 
  • monitoring of override patterns and outcomes 

AI provides a structured baseline. 

Governance determines how that baseline is applied. 

 

Accountability remains clinical 

AI does not remove accountability. It makes variation measurable. 

In governed systems, organisations can monitor: 

  • how often outcomes are escalated 
  • where overrides occur 
  • whether risk thresholds drift over time 

This visibility strengthens governance rather than weakening it. 

Final authority remains with the organisation and its clinical leadership. 

 

The future of defensible screening 

The evolution of fitness for work screening is not about replacing clinicians. 

It is about strengthening consistency, improving early risk visibility, and ensuring that decisions are explainable at scale. 

The most resilient models combine: 

  • structured data capture 
  • consistent decision intelligence 
  • accurate role-risk context 
  • defined governance frameworks 

For medical leaders, the opportunity is not automation. 

It is better control, clearer accountability, and stronger defensibility in environments where scale makes variation costly.
