How long does it take to complete the Carelever Comply Questionnaire?

Not long at all. Completing your Questionnaire will take approximately 10-15 minutes and can be done using an internet-enabled device (smart phone, tablet or computer). You can save your responses at any time and return back to the questionnaire, if you need to. 

Why can't I access the form?

Prior to accessing your questionnaire, you will be required to complete a two-factor authentication, where you will be sent a verification code to enter. Make sure you check your junk mail if you haven’t received the email verification into your inbox.

The form cannot be completed using Internet Explorer. Google Chrome, Safari or Microsoft Edge are the recommended browsers for completing the form.

If you are located outside of Australia, you will be unable to access the form. Please contact us so we can arrange for you to complete your Carelever Comply form. To Contact Us for assistance please complete our ‘Get in Touch’ form below.

I've forgotten my password / log-in details?

This happens to the best of us. Just click ‘forgot password’ and we’ll send you a verification code where you can reset your password and log back into your account. 

Where do I get my Immunisation information?

Great question. You can access your immunisation history statement from the Australian Immunisation Register by using:  

  • Your Medicare online account through myGov or the Medicare mobile app  
  • Call the Australian Immunisation Register (1800 653 809) and ask them to send it to you. 
I've had an email saying I need to provide further information. How do I do this?

If we’ve sent you an email requesting further information it means that you have not met the compliance requirements set by your University. This could be because the information you provided was out of date, too blurry for us to read, appears incorrect or incomplete.

Our Clinical Team will provide you with specific feedback on why they were unable to verify your information and what is required from you. This will be detailed in our email to you.

To fix this, you’ll need to log back into your Carelever Comply account and update your information where our email has indicated. Once this has been completed, you can re-submit the form and our team will review it. You’ll receive a confirmation email from us once your information has been verified.

How long does it take for the verification process?

It depends what we’re checking and many Universities have different requirements. Most verifications are processed within 5 business days. If you need your verification done sooner, get in touch with us and we will see if we can verify it sooner for you. 

How do you verify my information?

Our team takes the information that you’ve submitted and checks that it meets:

  • Your University’s requirements for clinical placement
  • That it matches the information held within the Australian Immunisation Register (if available on the register) once you have provided your Medicare Number.

The verification is completed by an experienced Clinical team member.

How does Carelever Comply keep my information secure?

There are few things more private than our health information so we take the job of looking after yours very seriously.
